5 EYEWITNESS NEWS - ALLSTATE MINNESOTA TEEN DISTRACTED DRIVING VIDEO CONTEST


OFFICIAL RULES


1.  HOW TO ENTER:  NO PURCHASE NECESSARY.  In order to enter, a viewer between the ages of 15 and 19, with a responsible adult's consent if under 18, must go online to www.kstp.com, complete the entry form and submit your Minnesota Teen Distracted Driving :30 second video.  Entries will be accepted from Monday, May 13, 2013, through 12:00 Noon CT on Thursday, June 13, 2013.  The :30 second video entry must contain a message that you wrote and created about the dangers of distracted driving, accompanied by your name, address, telephone number and email address.  Viewers must be between the ages of 15 and 19 to participate in this contest and must have a "responsible adult's" (who must be a parent or legal guardian and at least 25 years old) consent to enter if you are under 18.  5 Eyewitness News is not responsible for problems accessing the www.KSTP.com web site, technical or atmospheric conditions that disrupt the completion of the web site entry, or any other problems which might or might not result from this contest.  5 Eyewitness News is not responsible for lost, late, misdirected or incomplete video entries.  Limit, one entry per person; any person submitting more than one video entry, a video entry longer than :30 seconds or not between the ages of 15 and 19 may be disqualified.  

By entering this contest, each entrant (or his or her parent/legal guardian if entrant is under 18) is certifying that he or she is not a professional videographer, that the submitted video is his or her own original work, and that he or she has the right to allow the publication of the vide without restriction, and that 5 Eyewitness News’ use of the video will not infringe the rights of any third parties (e.g., copyrights, rights of publicity, etc.).

2.  HOW THE WINNER IS CHOSEN:  From all eligible entries received, 5 Eyewitness News staff members will choose the three top videos (3) based on the following judging criteria and percentages: (1) creativity (33.3%); (2) production elements (33.3%); and (3) quality of content (33.4%).  Then from Monday, June 17, 2013, through 11:59:59 p.m. CT on Thursday, July 4, 2013 ("Popular Voting Period"), all three videos will be available for viewing and voting through www.kstp.com, where viewers can vote for their favorite video submission.  Limit:  One (1) vote per person throughout the Popular Voting Period.  Upon conclusion of the Popular Voting Period, 5 Eyewitness News staff members will once again review the three (3) videos and will choose the winner based on the following judging criteria and percentages: (1) creativity (33%); (2) production elements (24%); (3) quality of content (33%) and (4) on-line voting results (10%). The submission with the highest score using the preceding judging criteria will be deemed the winner.  In the event of a tie, the entrant with the highest score in the quality of content criteria will be deemed the winner from among all tying participants.  The winner will be notified by telephone and/or email on or about Wednesday, July 10, 2013.

3.  CLAIMING YOUR PRIZE:  The winner will be asked to stop by the 5 Eyewitness News Studios, 3415 University Avenue, St. Paul, Minnesota 55114, during normal business hours (Monday-Friday, 8:30 a.m. to 5:00 p.m.) within ten (10) business days of the day he/she wins the prize to collect his/her prize.  When claiming a prize at the 5 Eyewitness News Studios, the winner must prove he or she is a qualified winner by presenting identification acceptable to the sponsors (e.g., state drivers license; state issued identification card; valid passport with address verification; or birth certificate with social security card and photo identification and address verification).  If a winner cannot prove he or she is the qualified winner, the prize will not be awarded.  Any unclaimed prizes remain the property of 5 Eyewitness News.

4.  PRIZES:  There will be one (1) winner in this contest. The winner will receive $1,000.00.  The responsible adult of the winner, if the winner is under 18, will be awarded the check from Allstate Insurance.  The prize is not transferable or exchangeable, and must be accepted in its entirety.  

A qualified winner must also meet the eligibility requirements as set forth in Paragraph 5 of these rules.  All income taxes and other taxes are the responsibility of the winner.  A 1099 form will be submitted to all appropriate taxing authorities. 

5.  ELIGIBILITY:  Individuals must be between the ages of 15 and 19 years of age, have the consent of a responsible adult to enter this contest if under 18, and be residents of the State of Minnesota.  Employees of Hubbard Broadcasting, Inc., KSTP-TV, LLC, 5 Eyewitness News and Allstate Insurance, including their subsidiaries, affiliates, parent or related companies, and their immediate families, are not eligible to enter.  

By entering this contest, each entrant is certifying that the submitted video is his or her own original work, and that he or she has the right to allow the airing of the video without restriction, and that KSTP-TV's use of the video will not infringe the rights of any third parties (e.g., copyrights, rights of publicity, etc.).

7.  CONTEST DEADLINE.  All entries must be received by 5 Eyewitness News no later than 12:00 Noon CT on Thursday, June 13, 2013.  
 
8.  RELEASE; CONSENT TO USE LIKENESS, VOICE AND ADDRESS:  The winner, or the responsible adult on behalf of the winner, must execute an affidavit of eligibility on behalf of himself/herself or the minor for which they are responsible.  The winner, or the responsible adult of the winner on behalf of the winner, (a) agrees to release and indemnify Hubbard Broadcasting, Inc., KSTP-TV, LLC, 5 Eyewitness News and Allstate Insurance (including their subsidiaries, affiliates, parent or related companies) from and against any and all liability, claims, damages and actions arising directly or indirectly out of or sustained in connection with the awarding, acceptance or use of the prize, and (b) consents to the use by 5 Eyewitness News for promotional purposes only of his/her likeness and/or voice, address (city and state) and his or her contest entry (which will be posted on the KSTP-TV website and aired on KSTP-TV) without any additional compensation.  By accepting the prize, the winner agrees to be bound by these official rules, including the above release.  By participating in this contest, each participant agrees to the application of these official rules.  

9.  WINNER LIST: 
For the name of the winner, send a separate self-addressed, stamped envelope before August 15, 2013, to Winners List, Allstate Minnesota Teen Distracted Driving Video Contest, 5 Eyewitness News, 3415 University Avenue, St. Paul, Minnesota 55114.

10. MISCELLANEOUS:  
Decisions of 5 Eyewitness News concerning any and all matters with respect to this contest shall be final.  5 Eyewitness News reserves the right to change these rules, as well as the terms and conditions of this contest, without notice.  5 Eyewitness News reserves the right to change the prize values at any time, or to substitute prizes of comparable value.  Contest void where prohibited or otherwise restricted by law.  Prizes are not transferable or exchangeable.  5 Eyewitness News is not responsible for printing errors in the contest materials or these official rules.

5 Eyewitness News, its subsidiaries and affiliated companies will conduct the contest substantially as described in these rules. Allstate Insurance Company has paid a promotional fee to have its name used in connection with this sweepstakes.  However, 5 Eyewitness News is responsible for the operation, management and all issues associated with this contest.  Any trade names, trademarks, or service marks shown as part of the materials associated with the contest away are the property of their respective owners.
