5 EYEWITNESS NEWS /MN LOTTERY PLATINUM PAYOUT DANCE GIVEAWAY
OFFICIAL RULES
1.HOW TO ENTER: NO PURCHASE NECESSARY. In order to enter, go to www.kstp.com, click on the “Minnesota Lottery Platinum Payout Dance Giveaway” icon, complete the entry form and submit a 20-30 second dance video depicting your reaction to winning one million dollars; entries will be accepted from Friday, January 15, 2010, through 11:59 p.m. on Friday, February 12, 2010. 5 Eyewitness News is not responsible for problems accessing the www.KSTP.com web site, technical or atmospheric conditions that disrupt the completion of the web site entry, or any other problems which might or might not result from this contest. Limit, one video entry per person, any person submitting more than one video entry or a video longer than 30 seconds may be disqualified; one winner per household.
2.HOW TO PLAY: In order to enter, go to www.kstp.com, click on the “Minnesota Lottery Platinum Payout Dance Giveaway” icon, complete the entry form and submit a 20-30 second dance video depicting your reaction to winning one million dollars; entries will be accepted from Friday, January 15, 2010, through 11:59 p.m. on Friday, February 12, 2010. From the eligible entries submitted, five (5) winners will be chosen by 5 Eyewitness staff members based on the dancer’s excitement in the video using creative, fun and stylish dance moves. The winners will be notified by telephone and/or e-mail on Tuesday, February 9, 2010.
3.CLAIMING YOUR PRIZE: The winners will be asked to stop by the 5 Eyewitness News Studios, 3415 University Avenue, St. Paul, Minnesota 55114, during normal business hours (Monday-Friday, 8:30 a.m. to 5:00 p.m.) within ten (10) days of the day he/she wins to claim their prize. 5 Eyewitness News is not responsible for problems resulting from the mailing of the prize to a winner’s home. When claiming a prize at the 5 Eyewitness News Studios, the winners must prove that he or she is a qualified winner by presenting identification acceptable to the sponsors (e.g., state drivers license; state issued identification card; valid passport with address verification; or birth certificate with social security card and photo identification and address verification). If a winner cannot prove he or she is the qualified winner, the prize will not be awarded. All unclaimed prizes remain the property of 5 Eyewitness News.
4.PRIZES: There will be five (5) winners in this contest. Each winner will receive thirty (30) $10 Mega Monopoly Minnesota Lottery scratch-off game pieces (value of each prize: $300.00); lottery pieces valid only at participating authorized Minnesota Lottery retailers in Minnesota. Scratch-off void if transferred, copied or duplicated in any way or transmitted via electronic media. This contest is subject to all policies, practices and procedures of the Minnesota Lottery. The Minnesota Lottery is not responsible for administering this contest or selecting the winners.
The prize is not transferable or exchangeable, nor can it be exchanged for merchandise, cash or credit, and must be accepted in its entirety. A qualified winner must also meet the eligibility requirements as set forth in Paragraph 5 of these rules. The winners are responsible for paying all applicable income taxes. A 1099 form may be submitted to all appropriate taxing authorities, and by signing the Prize Release, the winner accepts the prize, and even if the winner does not use the prize, he/she will be responsible for the applicable taxes.
Odds of winning are dependent on the total number of eligible entries received.
5.ELIGIBILITY: Individuals must be 18 years or older to be eligible to participate in this contest. Employees of Hubbard Broadcasting, Inc., KSTP-TV, LLC, 5 Eyewitness News and the Minnesota Lottery, including each of their parent companies, subsidiaries, affiliates, franchisees or related companies, and their immediate families, are not eligible to enter.
6.CONTEST DEADLINE. All entries must be received by 5 Eyewitness News no later than 11:59 p.m. on Friday, February 12, 2010. 5 Eyewitness News is not responsible for problems accessing the www.KSTP.com web site, technical or atmospheric conditions that disrupt the completion of the web site entry, or any other problems which might or might not result from this contest.
7.RELEASE; CONSENT TO USE LIKENESS, VOICE AND ADDRESS: The winners must execute an affidavit of eligibility. By participating in this contest and redeeming a prize, each winner hereby releases and agrees to hold Hubbard Broadcasting, Inc., KSTP-TV, LLC, 5 Eyewitness News and the Minnesota Lottery, and each of their respective parent companies, franchisees, affiliates, subsidiaries, local corporate advertising cooperatives, advertising/promotion agencies, and all of their respective directors, officers, employees, representatives and agents (collectively the “promotion entities”) from and against any and all liability, claims, damages and actions arising directly or indirectly out of or sustained in connection with the awarding, acceptance or use of the prize(s), and (b) consents to the use by the sponsors for promotional purposes only of his or her likeness, voice (the video may be shown on 5 Eyewitness News and/or posted on the website), and address (city and state) without any additional compensation. By accepting the prize, each winner agrees to be bound by these official rules, including the above release.
8.WINNERS LIST: For the names of the winners, send a separate self-addressed, stamped envelope before March 15, 2010 to Platinum Payout Dance Giveaway, 5 Eyewitness News, 3415 University Avenue, St. Paul, Minnesota 55114.
9.MISCELLANEOUS: Decisions of 5 Eyewitness News concerning any and all matters with respect to this contest shall be final. 5 Eyewitness News is not responsible for printing errors in the contest materials or these official rules. 5 Eyewitness News reserves the right to change these rules, as well as the terms and conditions of this contest, without notice. 5 Eyewitness News reserves the right to change the prize values at any time, or to substitute prizes of comparable value. If the sponsors determine in their sole discretion that the integrity or fairness of this contest has been corrupted in any manner, the sponsors reserve the right to terminate the contest and to select a winner from among entries legitimately received prior to termination. Contest void where prohibited or otherwise restricted by law.
