45TV - 12 DAYS OF CHRISTMAS 2013 SWEEPSTAKES

OFFICIAL RULES

1.HOW TO ENTER: This sweepstakes runs from 12:00 a.m. midnight on Monday, December 9, 2013, through 11:59:59 p.m. on Saturday, December 21, 2013. To participate in the sweepstakes, log on to the www.kstc45.com website and click on the “12 Days of Christmas” icon. A different prize will be revealed each day during this sweepstakes. Limit, one (1) entry per person per day for a total of thirteen (13) entries per person during the term of this sweepstakes; a person may only enter once for each of the 12 Days of Christmas; any person submitting more than thirteen (13) entries, or registering for a specific “Day of Christmas” more than one time, may be disqualified. Limit: one prize per person or household throughout the sweepstakes. 45TV is not responsible for problems accessing 45TV’s website or technical or atmospheric conditions that disrupt the completion of an entry, or any other problems which might or might not result from this sweepstakes.

2.HOW THE WINNERS ARE CHOSEN: On Monday, December 23, 2013, 45TV will randomly draw the winners names for each of the “12 Days of Christmas” drawing pools, and a total of 14 prizes will be awarded. The winners will be notified by email and/or telephone when necessary. Non-winning entries from any day will NOT be rolled over into subsequent drawing pools.

3.PRIZES:

a.Day 1 Prize - Unlocks on Monday, December 9, 2013: From all eligible entries received for this prize, one (1) name will be randomly drawn. The winner will receive a Fannie May Fine Chocolates in a chocolate tower. Value: $100.00.

b.Day 2 Prize - Unlocks on Tuesday, December 10, 2013: From all eligible entries received for this prize, one name (1) will be randomly drawn. The winner will receive $150.00 towards dinner or lunch at Masu Sushi and Robata Restaurants. Value: $150. Must be redeemed within six (6) months of winning.

c.Day 3 Prize - Unlocks on Wednesday, December 11, 2013: From all eligible entries received for this prize, one name will be randomly drawn. The winner will receive 1 lunch a month for a year at Wendy’s (up to $10.00 per lunch per month), awarded in the form of a Wendy’s Gift Card. Value: $120.00. Gift card’s are subject to the terms, conditions and restrictions as printed on the card.

d.Day 4 Prize - Unlocks on Thursday, December 12, 2013: From all eligible entries received for this prize, two names will be randomly drawn. Each winner will receive a gift basket from the Oilerie, complete with Sponsor-specified olive oils, balsamic vinegars, jams, and olives. Value: $200 each.

e.Day 5 Prize - Unlocks on Friday, December 13, 2013: From all eligible entries received for this prize, one name will be randomly drawn. The winner will receive Gift Cards from 15 Sponsor-specified shops located in the Downtown White Bear Lake Shoppes. Value of the prize: $1295. Each Gift Card is subject to the terms, conditions and restrictions printed on each Gift Card (including any expiration dates listed on the Cards, if any).

f.Day 6 Prize - Unlocks on Saturday, December 14, 2013: From all eligible entries received for this prize, one name will be randomly drawn. The winner will receive a $250.00 Gift Card to Harvest Grill. Value: $250. Gift Card is subject to the terms, conditions and restrictions printed on the Card.

g.Day 7 Prize - Unlocks on Sunday, December 15, 2013: From all eligible entries received for this prize, one name will be randomly drawn. The winner will receive a $250.00 Gift Card to ICC Wireless. Total value of prize: $250.00. Gift Card is subject to the terms, conditions and restrictions printed on the Card.

h.Day 8 Prize - Unlocks on Monday, December 16, 2013: From all eligible entries received for this prize, one name will be randomly drawn. The winner will receive hair cuts for one (1) year at Sport Clips (one hair cut per month), awarded in the form of a $250.00 Visa gift card. Value: $250. Gift card’s are subject to the terms, conditions and restrictions as printed on the card.

i.Day 9 Prize - Unlocks on Tuesday, December 17, 2013: From all eligible entries received for this prize, one (1) name will be randomly drawn. The winner will receive a $250.00 Gift Card to the 5-8 Club. Value: $250. Gift Card is subject to the terms, conditions and restrictions printed on the Card.

j.Day 10 Prize - Unlocks on Wednesday, December 18, 2013: From all eligible entries received for this prize, one name will be randomly drawn. The winner will receive a 2013-14 Season Pass to Wild Mountain in St. Croix Valley. Value: $399.00. This prize is subject to all application policies, practices and procedures of Wild Mountain.

k.Day 11 Prize - Unlocks on Thursday, December 19, 2013. From all eligible entries received for this prize, one name will be randomly drawn. The winner will receive a one-month car payment (up to $250.00) from White Bear Mitsubishi, awarded in the form of a $250.00 Visa gift card. Value: Up to $250.00. Gift card’s are subject to the terms, conditions and restrictions as printed on the card.

l.Day 12 Prize - Unlocks on Friday, December 20, 2013: From all eligible entries received for this prize, one name will be randomly drawn. The winner will receive a one (1) night hotel stay for winner and 3 guests (one room, quad occupancy), four (4) water park passes; and a 1-day unlimited play pass at Metropolis Resort and Chaos Water Park. Value: $259.00. This prize is subject to all application policies, practices and procedures of Metropolis Resort and Chaos Water Park.

m.Day 13 Prize – Unlocks Saturday, December 21, 2013: From all eligible entries received for this prize, one name will be randomly drawn. The winner will receive a Dyson DC35 vacuum from Warners’ Stellian. Value: $250.00. Any warranties and guarantees relating to these prizes are strictly limited to the manufacturer’s.

The prizes are not transferable or exchangeable, nor can they be exchanged for cash or credit. No change or credit will be issued for any unused portion of a certificate. The certificates are subject to the policies and procedures of the establishment at which the gift certificate is redeemed. Each certificate may have an expiration date. Any warranties and guarantees relating to these prizes are strictly limited to the manufacturer’s. A qualified winner must also meet the eligibility requirements as set forth in Paragraph 5 of these rules. The winners are responsible for paying all applicable income taxes. A 1099 form may be submitted to all appropriate taxing authorities, and by signing the Prize Release, the winner accepts the prize, and even if the winner does not use the prize, he/she will be responsible for the applicable taxes.

Odds of winning are dependent on the number of eligible entries received for each Day of Christmas.

4.CLAIMING YOUR PRIZE: Each winner must claim his/her prizes in person at KSTC.TV, 3415 University Avenue, St. Paul, Minnesota 55114, during normal business hours (Monday-Friday, 9:00 a.m. to 5:00 p.m.) within ten (10) days of the day he or she wins to verify that he/she is the qualified winner. Failure to comply with this deadline shall be deemed to be the winner’s forfeiture of his or her right to claim the prize. The winner must prove he or she is a qualified winner by presenting identification acceptable to 45TV (e.g., state drivers license; state issued identification card; valid passport with address verification; or birth certificate with social security card and photo identification and address verification). If a winner cannot prove he or she is a qualified winner, another name will be randomly drawn from eligible entries.

5.ELIGIBILITY: Individuals must be 18 years or older to be eligible to win. Employees of KSTC-TV, LLC, Hubbard Broadcasting, Inc., Fannie May, Masu Sushi and Robata Restaurants, Wendy’s, The Oilerie, Downtown White Bear Lake Shoppes, Harvest Grill, ICC Wireless, Sport Clips, 5-8 Club, Wild Mountain, White Bear Mitsubishi, Metropolis Resort and Warner’s Stellian, including their subsidiaries, affiliates, parent or related companies, and their immediate families, are not eligible to enter. Any one participating in this sweepstakes or claiming a prize in this sweepstakes in violation of these rules will be disqualified from participating in 45TV sweepstakes or contests for a period of one year.

6.RELEASE; CONSENT TO USE LIKENESS, VOICE AND ADDRESS: Each winner must execute an affidavit of eligibility. Each winner (a) agrees to release and indemnify KSTC-TV, LLC, Hubbard Broadcasting, Inc., Fannie May, Masu Sushi and Robata Restaurants, Wendy’s, The Oilerie, Downtown White Bear Lake Shoppes, Harvest Grill, ICC Wireless, Sport Clips, 5-8 Club, Wild Mountain, White Bear Mitsubishi, Metropolis Resort and Warner’s Stellian (including their officers, directors, employees and subsidiaries, affiliates, parent or related companies) from and against any and all liability, claims, damages and actions arising directly or indirectly out of or sustained in connection with the awarding, acceptance or use of the prize(s), (b) agrees that no sponsors or agents have made any representation, warranty or guaranty relative to the prize, and (c) consents to the use by the sponsors of this sweepstakes for promotional purposes only of her likeness and/or voice, and address (city and state) without any additional compensation. By participating in this sweepstakes, each participant agrees to the application of these official rules. By accompanying the winner, a winner’s guest agrees to be bound by the terms of these official rules, including the preceding release.

7.WINNER LIST: For the names of the winners, send a separate self-addressed, stamped envelope before January 31, 2014, to Winners List, 12 Days of Christmas Sweepstakes, 45TV, 3415 University Avenue, St. Paul, Minnesota 55114.

8.MISCELLANEOUS:

Decisions of the sponsors concerning any and all matters with respect to this sweepstakes shall be final. The sponsors reserve the right to change these rules, as well as the terms and conditions of this sweepstakes, without notice. If the sponsors determine in their sole discretion that the integrity or fairness of this sweepstakes has been corrupted in any manner, the sponsors reserve the right to terminate the sweepstakes and to select a winner from among entries legitimately received prior to termination. The sponsors reserve the right to change the prize values at any time, or to substitute prizes of comparable value. Sweepstakes void where prohibited or otherwise restricted by law. The sponsors are not responsible for printing errors in the sweepstakes materials or these rules.
