HOW TO ENTER:
NO PURCHASE NECESSARY. In order
to enter, a viewer between the ages of 16 and 18, with a responsible adult's
consent, must go online to KSTP.com, complete the entry form and submit your
Minnesota Teen safe Driving :30 second video.
Entries will be accepted from Monday, October 12, 2009, through 11:59:59
on Sunday, November 8, 2009. The :30
second video entry must contain a teen safe driving message that you wrote and
created, accompanied by your name, address, telephone number and email
address. Viewers must be between the
ages of 16 and 18 to participate in this contest and must have the
"responsible adult's" (who must be a parent or guardian and at least
25 years old) consent to enter. 5
Eyewitness News is not responsible for problems accessing the www.KSTP.com web
site, technical or atmospheric conditions that disrupt the completion of the
web site entry, or any other problems which might or might not result from this
contest. 5 Eyewitness News is not
responsible for lost, late, misdirected or incomplete video entries. Limit, one entry per person; any person
submitting more than one video entry, a video entry longer than :30 seconds or
not between the ages of 16 and 18 may be disqualified.
2. HOW
TO PLAY: In order to enter, a

2009. The :30 second video entry must

begin Monday, November 9 and continue through Wednesday, November 18, 2009.
From all eligible entries received and in conjunction with online voting
results, a panel of judges comprised of 5 Eyewitness News and Twin Cities Live
staff member will select the winner in their sole and absolute discretion,
based on creativity and the effectiveness of the safe driving message. The
winner will be notified by telephone and/or e-mail on Friday, November 20,
2009. Contest open to residents of Minnesota only.
3. HOW
THE WINNER IS CHOSEN: Online voting
at kstp.com runs Monday, November 9 through Wednesday, November 18, 2009.
Viewers will be asked to go to the www.kstp.com to vote for the video they
believe deserves to win this contest.
Viewers may only vote once during the time that the videos are posted on
the website. From all eligible entries
received and in conjunction with online voting results, a panel of judges
comprised of 5 Eyewitness News and Twin Cities Live staff member will select
the winner in their sole and absolute discretion, based on creativity and the
effectiveness of the safe driving message. The winner will be notified by
telephone and/or email on Friday, November 20, 2009.
4. CLAIMING
YOUR PRIZE: The winner will be asked to stop by the 5
Eyewitness News Studios, 3415 University Avenue, St. Paul, Minnesota 55114,
during normal business hours (Monday-Friday, 8:30 a.m. to 5:00 p.m.) within ten
(10) business days of the day he/she wins the prize to collect his/her prize.
When claiming a prize at the 5 Eyewitness News Studios, the winner must prove
he or she is a qualified winner by presenting identification acceptable to the
sponsors (e.g., state drivers license; state issued identification card;
valid passport with address verification; or birth certificate with social security
card and photo identification and address verification). If the winner cannot prove he or she is the
qualified winner, the prize will not be awarded. Any unclaimed prize remains the property of 5 Eyewitness News.
5. PRIZE: There
will be one (1) prize winner. The responsible adult of the winner will
be awarded the $1,000.00 check from Allstate Insurance (Value: $1000.00)
The prize is not transferable or exchangeable, and must be accepted in
its entirety. The winner may be asked
to appear on Twin Cities Live to show off his/her winning safe driving video.
A qualified winner must also meet the eligibility requirements as set
forth in Paragraph 6 of these rules.
All income taxes and other taxes are the responsibility of the
winner. A 1099 form will be submitted
to all appropriate taxing authorities.
Odds of winning are dependent on the number of eligible entries
received.
6. ELIGIBILITY: Individuals must be between the ages of 16
and 18 years of age, have the consent of a responsible adult to enter this
contest, and be residents of the State of Minnesota. Employees of Hubbard Broadcasting, Inc., KSTP-TV, LLC, 5
Eyewitness News and Allstate Insurance including their subsidiaries,
affiliates, parent or related companies, and their immediate families, are not
eligible to enter.
7. CONTEST
DEADLINE. All entries must be
received by 5 Eyewitness News no later than 11:59:59 on Sunday, November 8,
2009. 5 Eyewitness News is not
responsible for problems accessing the www.KSTP.com web site, technical or
atmospheric conditions that disrupt the completion of the web site entry, or
any other problems which might or might not result from this contest.
8. RELEASE;
CONSENT TO USE LIKENESS, VOICE AND ADDRESS: The responsible adult must execute an affidavit of eligibility on
behalf of himself/herself and the minor for which they are responsible. The winner (a) agrees to release and
indemnify Hubbard Broadcasting, Inc., KSTP-TV, LLC, 5 Eyewitness News, and Allstate
Insurance (including their subsidiaries, affiliates, parent or related
companies) from and against any and all liability, claims, damages and actions
arising directly or indirectly out of or sustained in connection with the
awarding, acceptance or use of the prize, and (b) consents to the use by 5
Eyewitness News for promotional purposes only of his/her likeness and/or voice,
address (city and state) and his or
her contest entry (which may be posted on the KSTP-TV website) without
any additional compensation. By
accepting the prize, the winner agrees to be bound by these official rules,
including the above release.
9. WINNER
LIST: For the name of the winner,
send a separate self-addressed, stamped envelope before December 30, 2009, to
Winners List, Minnesota Teen safe Driving Contest, 5 Eyewitness News, 3415
University Avenue, St. Paul, Minnesota 55114.
10. MISCELLANEOUS:
Decisions
of 5 Eyewitness News concerning any and all matters with respect to this
contest shall be final. 5 Eyewitness
News reserves the right to change these rules, as well as the terms and
conditions of this contest, without notice.
5 Eyewitness News reserves the right to change the prize values at any
time, or to substitute prizes of comparable value. Contest void where prohibited or otherwise restricted by law. Prizes are not transferable or
exchangeable. 5 Eyewitness News is not
responsible for printing errors in the contest materials or these official
rules.
Eyewitness 5 News, its subsidiaries and affiliated
companies will conduct the contest substantially as described in these rules. Allstate Insurance Company has paid a promotional fee to have its name
used in connection with this sweepstakes. However, Eyewitness 5 News is
responsible for the operation, management and all issues associated with this
contest. Any trade names, trademarks, or service marks shown as part of
the materials associated with the contest away are the property of their
respective owners.
