1.HOW TO ENTER: NO PURCHASE NECESSARY. In order to enter, from 3:00 p.m. on Monday, April 16, 2012, through 11:59:59 p.m. on Friday, May 4, 2012, go to the www.twincitieslive.com website, click on the "contest” icon and then click on the "Send My Mom to Mexico" icon and complete the entry form, which includes stating in 500 words or less what is special about your Mom and why she deserves a makeover and a trip, and submit a photograph of your Mom. KSTP-TV is not responsible for problems accessing the website or completing the entry, technical or atmospheric conditions that disrupt the completion of the entry, or any other problems which might or might not result from this contest. Any person submitting more than one entry, an entry containing more than 500 words or an entry without a photograph attached may be disqualified.
2.HOW TO PLAY/HOW THE WINNER IS CHOSEN: Enter as explained in Paragraph 1 above. From all eligible entries submitted, a panel of judges comprised of staff members from Twin Cities Live, in their sole discretion, will determine the winner based on what you say is special about your Mom (49%) and the reason why you state your Mom needs a makeover and a trip (51%). In the event of a tie, the contestant with the highest score in the why your Mom needs a makeover and a trip category will be deemed the winner from among all tying contestants. The winner will be chosen on Monday, May 7, 2012; the person who nominated the "Mom" will be contacted by Twin Cities Live staff via email and telephone. The winner will be surprised "live" on Twin Cities Live on Thursday, May 10, 2012. The winner and the nominee must be available on Thursday, May 10, 2012, and Friday, May 11, 2012, between 2:00 and 4:00 p.m. The winner must also be available from 9:00 a.m. to 2:00 p.m. on Friday, May 11, 2012, to have her makeover performed.
3.CLAIMING YOUR PRIZE: The winner must claim his/her prize in person at the KSTP-TV Studios, 3415 University Avenue, St. Paul, Minnesota 55114, during normal business hours (Monday-Friday, 9:00 a.m. to 5:00 p.m.) within twelve (12) hours of the time she is first notified. Failure to comply with this deadline shall be deemed to be the winner’s forfeiture of his or her right to claim a prize and another winner will be chosen. The winner must present identification acceptable to KSTP-TV (e.g., state drivers license; state issued identification card; valid passport with address verification; or birth certificate with social security card and photo identification and address verification). If a qualifier winner cannot prove he or she is qualified to accept the prize, the prize will not be awarded. Any unclaimed prize remains the property of Twin Cities Live.
4.PRIZE: There will one (1) winner in this contest. The winner will be the Mom who has been nominated. The winner will receive the following:
a.A make-over consisting of the following:
i.Haircut, hair color and make-up application from New Reflections Salon on Friday, May 11, 2012; value: $250.
ii.A $150 gift card to Ridgedale Center. The winner will need to go to Ridgedale Center on Friday, May 11, 2012, where she will meet with a marketing representative of Ridgedale Center who will help the winner choose a new outfit to be worn on Twin Cities Live that day. The winner will have the option of either using her Ridgedale Center card to purchase the outfit she chooses that day, or the winner can just borrow the outfit for the show and then use the gift card at another time. The gift card is subject to the policies, practices and procedures of Ridgedale Center and does not have an expiration date.
b.A certificate from Delta Vacations redeemable for a trip for two (2) to any Delta Vacations destination in Mexico, which includes roundtrip airfare from Minneapolis/St. Paul, Minnesota (value up to $900), and $1,200 to spend on hotel accommodation at the hotel of winner's choice for a minimum of two (2) nights (one room, double occupancy. Total value of the prize is $2,100.00. The trip must be booked by December 31, 2012, and travel must be completed by February 28, 2013. All other expenses (including, without limitation, ground transportation, personal expenses, telephone charges, meals and beverages, and all federal, state and local taxes (including applicable income taxes) and fees incurred are the winner's/guest's sole responsibility. The vacation package is non-refundable and non-transferable. Tickets and hotel certificates issued in conjunction with this contest have zero cash value. Travel is permitted on designated flights operated by Delta Air Lines, KLM, Compass, Pinnacle, KLM City Hopper, ASA, Freedom, Comair, SkyWest, Chautauqua or Shuttle America. Travel via other Delta designated affiliated partner flights is not permitted. Travel is not permitted on the following dates: 2012 - November 17-23, December 18-31; 2013 - March 20-31, November 21-27, December 18-31. Cannot be applied towards previously purchased trip. This certificate may not be combined with any other coupons, certificates, discount, bonus, infant fare, upgrade, SkyMiles air travel award or promotional offer/tickets. This certificate is not valid with Miscellaneous Charge Orders or prepaid tickets. Trip certificate is not applicable to Ski, Dive, Asia Escorted Tours, Globus family of brands including, Globus, cosmos, Monograms and Avalon. The certificate cannot be sold or transferred. Purchased certificates are subject to confiscation and you will not be allowed to travel. Only original certificates will be accepted. Certificates will not be replaced if lost. Void if altered. Travel must be roundtrip. Reservations must be made and ticket issued prior to departure. Booking and travel dates must fall into the range as specified on the front of the certificate. Delta Vacations must issue all travel documentation. Certificate may only be applied towards number of passengers specified on the certificate; passengers must travel on the same itinerary and must be booked through Delta Vacations. Any unused portion of the certificate will not be refunded and cannot be used towards another booking or another service component. Travel agent commission is not paid/earned on a free trip value, taxes and fees. If booking exceeds certificate value, the winner is responsible for any balance due and must be paid in full at time of booking. Hotel taxes, state and federal income taxes may apply and are the sole responsibility of the recipient. Changes to reservations, once made, are not permitted, Checked luggage fees may apply; fees are charged each way, and are subject to change. A per ticket surcharge will apply for paper tickets issued in markets where electronic ticketing is available. Substitutions will not be allowed. Space is subject to availability. SkyMiles mileage credit will be awarded. Flight schedules are subject to change without notice. Delta Vacations is the final authority on the interpretation of these terms and reserves the right to changes these terms and conditions without prior notice. Standard Delta Vacations terms and conditions apply, and are outlined during the booking process; other airline, hotel or vendor restrictions may apply. If a winner and/or his/her guest misses the initial flight as scheduled, the winner will forfeit the prize. If the winner and/or his/her guest is not present for the designated return flight, he/she is solely responsible for his/her return to Minnesota. IMPORTANT NOTICE REGARDING NEW PASSPORT REQUIREMENTS FOR INTERNATIONAL TRAVEL: As of January 23, 2007, all international travelers must present a government issued passport for U. S. Customs re-entry after air, land or sea travel. For more information please visit the State Department's consular website www.travel.state.gov or the U.S. Department of Homeland Security's website www.dhs.gov. It is the traveler's responsibility to provide proper documentation. This trip is subject to all applicable airline and hotel policies, practices and procedures.
A qualified winner must also meet the eligibility requirements as set forth in Paragraph 5 of these rules. The winner is responsible for paying all applicable income taxes. A 1099 form will be submitted to all appropriate taxing authorities, and by signing the Prize Release, each winner accepts the prize, and even if the winner does not use the prize/trip, he/she will be responsible for the applicable taxes.
5.ELIGIBILITY: Individuals must be 18 years or older, or have written permission to enter from a parent or legal guardian, to be eligible to participate in this contest, and must be and residents of Minnesota or Wisconsin. Employees of Hubbard Broadcasting, Inc., KSTP-TV, LLC, Delta Vacations, MLT Vacations, Ridgedale Center and New Reflections, including their subsidiaries, affiliates, parent or related companies, and their immediate families, are not eligible to enter. In order to be eligible to win a prize in this contest, you cannot have won and accepted any prize from Twin Cities Live within 30 days prior to the awarding of a prize to you in this contest, or won and accepted any prize or combination of prizes with a total stated value of $1,000.00 or more from Twin Cities Live within 365 days prior to the awarding of a prize to you in this contest. The winning mother must be 21 years of age or older.
By entering this contest, each entrant is certifying that he or she is not a professional photographer, that the submitted photograph is his or her own original work, and that he or she has the right to allow the publication of the photograph without restriction, and that KSTP-TV's use of the photograph will not infringe the rights of any third parties (e.g., copyrights, rights of publicity, etc.).
6.CONTEST DEADLINE. All entries must be received by Twin Cities Live no later than 11:59:59 p.m. on Friday, May 4, 2012.
7.RELEASE; CONSENT TO USE LIKENESS, VOICE AND ADDRESS: The winner must execute an affidavit of eligibility. Each person entering this contest (a) agrees to release and indemnify Hubbard Broadcasting, Inc., KSTP-TV, LLC, Delta Vacations, MLT Vacations, Ridgedale Center and New Reflections (including their subsidiaries, affiliates, parent or related companies) from and against any and all liability, claims, damages and actions arising directly or indirectly out of or sustained in connection with the awarding, acceptance or use of the prize(s), and (b) consents to the use by KSTP-TV for promotional purposes only of his likeness and/or voice, address (city and state) and his or her contest entry in any manner or medium without any additional compensation. By accepting the prize, the winner and his/her guest agree to be bound by these official rules, including the above release.
9.WINNER LIST: For the name of the winner, send a separate self-addressed, stamped envelope before June 15, 2012, to Winners List, Twin Cities Live, Send My Mom to Mexico Contest, 3415 University Avenue, St. Paul, Minnesota 55114.
10.MISCELLANEOUS:
Decisions of KSTP-TV concerning any and all matters with respect to this contest shall be final. KSTP-TV reserves the right to change these rules, as well as the terms and conditions of this contest, without notice. KSTP-TV reserves the right to change the prize values at any time, or to substitute prizes of comparable value. Contest void where prohibited or otherwise restricted by law. Prizes are not transferable or exchangeable. KSTP-TV is not responsible for printing errors in the contest materials or these official rules.
