5 EYEWITNESS NEWS GAS GIVEAWAY IN MAY OFFICIAL RULES
1. HOW TO ENTER: NO PURCHASE NECESSARY.
In order to enter, you must log on to the www.kstp.com website, and then choose the keyword "Gas Contest". You must complete all of the required information on the entry form (name, date of birth, email address, daytime telephone number and any other required fields) and then submit your entry by clicking on the "SUBMIT" button from 12:01:00 a.m. on Monday, April 16, 2012, through 11:59:00 p.m. on Friday, May 18, 2012.
5 Eyewitness News is not responsible for problems accessing the www.kstp.com website, technical or atmospheric conditions that disrupt the completion of the website entry, or any other problems which might or might not result from this contest. Limit, one (1) entry per person; any person submitting more than one entry may be disqualified; one (1) daily winner per household. All non-winning daily entries will be rolled into subsequent daily drawings, and each daily prize winner is eligible to win the grand prize.
2. HOW TO /PLAY:
a. Daily Winners. This portion of the contest is a "watch and win" contest. Enter pursuant to Paragraph 1 above. In order to play, you must watch 5 Eyewitness News at 10:00 p.m. each Sunday through Thursday, beginning Sunday, April 29, 2012, through Thursday, May 24, 2012. 5 Eyewitness News will randomly draw one (1) entry each Sunday through Thursday during the contest from all eligible entries received, and display the person's name and city during the newscast between 10:00 p.m. and 10:30 p.m. The person whose name is displayed will have until 10:35 p.m. the day his/her name is displayed to call 5 Eyewitness News at (651) 642-4629 to claim his/her prize. 5 Eyewitness News will not accept collect calls. The person calling in to the station to claim the prize must be the person whose name is displayed; substitute callers (e.g. friends, relatives, co-workers, etc.) are not allowed, except in the case of a disability that prevents the person whose name is displayed from calling himself or herself, and will result in disqualification at the sole discretion of 5 Eyewitness News. If the person whose name is displayed between 10:00 p.m. and 10:30 p.m. does not call by the end of the newscast, there will not be a prize winner for that day, and that day's prize shall be forfeited and not awarded. When calling in to the station, the caller must verify his/her name, birth date and address. There will be up to nineteen (19) daily winners.
b. Grand Prize Winner: Enter pursuant to Paragraph 1 above. From all eligible entries received, one name will be randomly drawn at the 5 Eyewitness News studios on Thursday, May 24, 2012. The winner's name will be announced on 5 Eyewitness News during the 10:00 p.m. newscast on Thursday, May 24, 2012; the winner will also be contacted by telephone on Friday, May 25, 2012.
3. PRIZES:
a. Daily Prizes: There will be up to nineteen (19) daily winners. Each person whose name is displayed and calls in within the allotted time period as set forth in Paragraph 2 above will win a $500 Visa gift card. The gift cards are subject to the policies, practices and procedures of Visa.
b. Grand Prize Winner: There will be one (1) grand prize winner. The winner will receive a $3,000 Visa gift card. The gift card is subject to the policies, practices and procedures of Visa.
A qualified winner must meet the eligibility requirements as set forth in Paragraph 5 of these rules. Each winner is responsible for paying any applicable taxes, including, without limitation, income taxes. A 1099 form may be submitted to all appropriate taxing authorities.
Odds of winning are dependent on the number of eligible entries received in this contest.
4. CLAIMING YOUR PRIZE: Each winner must claim his/her prize in person at the 5 Eyewitness Studios, 3415 University Avenue, St. Paul, Minnesota 55114, during normal business hours (Monday-Friday, 9:00 a.m. to 5:00 p.m.) within five (5) days after the day he or she wins. This is to verify that the prize is being given to the qualified winner. Failure to comply with this deadline shall be deemed to be the winner's forfeiture of his or her right to claim a prize. The winner must present identification acceptable to 5 Eyewitness News (e.g., state drivers license; state issued identification card; valid passport with address verification; or birth certificate with social security card and photo identification and address verification). If a winner cannot prove he or she is qualified to accept the prize, the prize will not be awarded. All unclaimed prizes remain the property of 5 Eyewitness News.
5. ELIGIBILITY: Individuals must be 21 years or older to be eligible to win a prize in this contest. Employees of Hubbard Broadcasting, Inc. and KSTP-TV, LLC, including their subsidiaries, affiliates, parent or related companies, and their immediate families, are not eligible to enter. Any one participating in this contest or claiming a prize in this contest in violation of these rules will be disqualified from participating in 5 Eyewitness News contests for a period of one year.
6. RELEASE; CONSENT TO USE LIKENESS, VOICE AND ADDRESS: Each winner agrees to release and indemnify Hubbard Broadcasting, Inc. and KSTP-TV, LLC (including their officers, directors, employees and subsidiaries, affiliates, parent or related companies) from and against any and all liability, claims, damages and actions arising directly or indirectly out of or sustained in connection with the awarding, acceptance or use of the prize, and (b) consents to the use by the sponsors of this contest for promotional purposes only of his or her likeness and/or voice and address (city and state) without any additional compensation. By accepting the prize, each winner agrees to be bound by these official rules, including the above release.
7. WINNER LIST: For the names of the winners, send a separate self-addressed, stamped envelope before June 30, 2012, to Winners List, Gas Giveaway in May, 5 Eyewitness News, 3415 University Avenue, St. Paul, Minnesota 55114.
8. MISCELLANEOUS:
Decisions of the sponsors concerning any and all matters with respect to this contest shall be final. The sponsors reserve the right to change these rules, as well as the terms and conditions of this contest, without notice. The sponsors reserve the right to change the prize values at any time, or to substitute prizes of comparable value. Contest void where prohibited or otherwise restricted by law. The sponsors are not responsible for printing errors in the contest materials or these rules. If the sponsors determine in their sole discretion that the integrity or fairness of this contest has been corrupted in any manner, the sponsors reserve the right to terminate the contest and to select a winner from among entries legitimately received prior to termination. KSTP-TV is not responsible for delays attributable to webcasting, telephone line unavailability, busy signals on the contest line, operator interference, technical or atmospheric conditions that disrupt the completion of the phone call, telephone system malfunctions, or any other telephone problems which may or may not result from this contest.
